No repair work will begin until the customer has reviewed and approved the written quotation. Please include the completed Repair Request Form inside your shipment to avoid processing delays.

Download the Customer Repair Request & Service Authorization Form (PDF).
Step 2
Complete all required customer, equipment, part, and problem information.
Step 3
Review and sign the Customer Authorization section. Electronic and handwritten signatures are accepted. Printing is optional.
Step 4
Email the completed form to tech@motorcityharnessmedics.com. If you plan to ship an item, you may also include a printed copy inside your package for reference.
Step 5
After reviewing your submission, Motor City Harness Medics will contact you with your Repair Reference Number (if applicable), shipping instructions, and a written repair estimate before any repair work begins.
Next Steps
Once we review your electronic form, we will provide a Repair Reference Number and specific shipping logistics. We will then issue a comprehensive written estimate for your approval before any repair work commences. Printing the form is entirely optional for your own records or if you prefer to include a paper copy with your shipment.

PACKAGING & SHIPPING INSTRUCTIONS
• Use a sturdy double-walled shipping box.• Coil wire harnesses loosely and avoid sharp bends.• Add at least 2 inches of protective padding on all sides.• Secure the item so it cannot move inside the box.• Include your Repair Reference Number, if one has been assigned.
